The Social Work Team provides support to people during their time with Horizons. Social work support is provided to all age groups that avail of services within the organisation; from early intervention to school age to adult services.
Social Work Team
Social workers are usually the first point of contact which families have with Horizons following a diagnosis. The team work to maximise the potential of individuals, families, communities and society to ensure the people we support are free to live a life which they choose.
Some of the key roles of the social worker include:
- To support people we support and their families after the diagnosis of a disability.
- To assist in the planning and coordination of services.
- To empower people we support to access benefits and entitlements.
- To promote people we support’s own control and involvement in their lives, communities, and services.
- To work with other disciplines and agencies to ensure the needs of the people we support are met.
